INFORMATION IN APPLYING FOR ISSUANCE OF LGU PERMIT CERTIFICATION
In situations where an LGU shall require a COC/LTO as a prerequisite to a Mayor’s or Business Permit, a certification may be issued to a new Retail Outlet solely for that purpose of obtaining the Mayor’s or Business Permit.

Checklist of Requirements
The following are the related forms (application forms, annexes and references) for each industry participant:
- Letter of Intent indicating business name, owner, business address, contact information, and email address.
- DOE-VFO-EIMD-QF-00 (DOE Clearance of No Pending Case)
- For LFRO applicants:
- For MRO applicants:
- For TERO applicants:

- For LTO applicants: Refer to Citizen Charter for Issuance of License to Operate (LTO) for Liquified Gas (LPG) Establishments
- Valid supply Agreement
- For LFRO applicants:
- Notarized Statement of Facility PNS Compliance
- For MRO applicants:
- Notarized certification issued by the Engineering Procurement Construction Contractor and dealer/retailer
- Copy of Manufacturer's safety certification on the storage tank
- For TERO applicants:
- Written notice to OIMB with endorsement from LGU or LDRRMC
- Notarized Certification of Compliance to “Revised Retail Rules”
- For TSRO applicants:
- Copy of Endorsement/Certification from the concerned LGU addressing the proliferation of “bote-bote” retailing in the area
- Copy of Certification that equipment is manufactured in accordance with the product safety standards
- For LTO applicants: Refer to Citizen Charter for Issuance of License to Operate (LTO) for Liquified Gas (LPG) Establishments
- Copy of List of dispensing pumps/hoses and storage tank/s with corresponding capacity/ies
- Scaled Layout Plan
- Latest photographs showing the establishment, required labels, price display board, and safety & informational signs.
- Official Receipt of Payment
You may download the forms above, together with the required supporting documents, for submission. Please be informed that the receipt of application depends on the completeness of requirements, use of right forms, filling-up of required entries or information. Only after the required documents have been received, including the Legal Clearance for No Pending Case and Payment of the Required Fees, shall the official processing of application commence. The processing of the LGU Permit is in conformance to our Citizen’s Charter for the required number of days of processing.
Note: The Certification, however, does not allow the applicant to operate without a Certificate of Compliance / License to Operate. The certification is only valid for thirty (30) days from the date of issuance, within which the above-mentioned permits shall be submitted, or the certification is deemed to be automatically revoked.
With the submission of the requested permit, the applicant is also required to resubmit the Affidavit of Undertaking noting the compliance in the issuance of the Mayor's Business Permit.
