Visayas Field Office - License to Operate

INFORMATION IN APPLYING FOR REGISTRATION AND ISSUANCE OF LICENSE TO OPERATE QUALIFIED LPG INDUSTRY PARTICIPANTS IN THE VISAYAS

In compliance with the DOE Department Circular DC2022-11-0037, or the Guidelines in the Registration and Issuance of License to Operate to Qualified LPG Participants, all LPG Industry facilities and participants operating in the Visayas shall apply for required registration and License to Operate. The following industry players shall be under the jurisdiction of the DOE – Visayas Field Office:

  1. LPG Refilling Plants
  2. LPG Independent Haulers, in cylinder or cartridge
  3. LPG Dealers
  4. LPG Retailers
  5. Auto-LPG Dispensing Stations
  6. Centralized LPG Piping System
  7. LPG Bulk Consumers

The following are the related forms (application forms, annexes and references) for each industry participant:

  1. Refilling Plant
    1. Registration to Prior to Commencement of Facility Construction (Annex 1);
    2. Registration to Fill Third-Party Trademark or Tradename (Annex 3);
    3. Registration as Authorized LPG Delivery Motor Vehicles and Marine Vessels (Annex 7)
    4. License to Operate ( Annex E,   Annex E1 to E4 ).
  2. LPG Independent Haulers, in cylinders or cartridges
    1. Registration as Authorized LPG Delivery Motor Vehicles and Marine Vessels (Annex 7)
    2. License to Operate ( Annex C,   Annex C1 to C4 ).
  3. LPG Dealers
    1. Registration for Authority to Sell LPG-Filled Pressure Vessel (Annex 4);
    2. Registration as Authorized LPG Delivery Motor Vehicles and Marine Vessels (Annex 7)
    3. License to Operate ( Annex F,   Annex F1 to F5 ).
  4. LPG Retailers
    1. Registration for Authority to Sell LPG-Filled Pressure Vessel (Annex 4);
    2. Registration as Authorized LPG Delivery Motor Vehicles and Marine Vessels (Annex 7)
    3. License to Operate ( Annex G,   Annex G1 to G5 ).
  5. Auto-LPG Dispensing Stations
    1. License to Operate ( Annex H,   Annex H1 to H4 ).
  6. Centralized LPG Piping System
    1. Registration to Prior to Commencement of Facility Construction (Annex 1);
    2. License to Operate ( Annex I,   Annex I1 to I4 ).
  7. LPG Bulk Consumers
    1. Registration for Bulk Consumers (Annex 6);
    2. Criteria for a Bulk Consumer (Annex 6-A);

You may download the forms above for submission, together with the supporting required. Please be informed that receipt of application depends on the completeness of requirements, use of right forms, filling-up of required entries or information. Only after the required documents have been received, including the Legal Clearance for No Pending Case and Payment of the Required Fees, shall the official processing of application commence. The processing of the LTO is in conformance to our Citizen’s Charter for the required number of days of processing.
